How UTI is diagnosed

Diagnosing a urinary tract infection (UTI) involves several steps to accurately identify the presence and cause of the infection:​

  • Symptom assessment: Initial evaluation includes discussing symptoms like frequent urination, burning sensation during urination, pelvic or lower abdominal pain, cloudy or strong-smelling urine, and sometimes fever
  • Urinalysis: A common initial test that checks for white blood cells, red blood cells, nitrites (indicating gram-negative bacteria like E. coli), and signs of infection.
  • Urine culture: If needed, a urine culture is performed to identify the specific bacteria causing the infection and to determine which antibiotics the bacteria are sensitive to. This helps tailor antibiotic treatment effectively
  • Imaging tests: For recurrent, complicated, or severe UTIs, imaging studies such as ultrasound or CT scans, or MRI may be used to check the urinary tract for abnormalities.​
  • Cystoscopy: In certain cases, a cystoscope is used to examine the bladder and urethra for structural issues.​

Recent advances also include molecular and rapid diagnostic tests, but the traditional approach remains based on symptoms, urinalysis, and urine culture.

Treatment for UTIs is tailored based on the infection’s severity, location, and patient-specific factors:​

  • Antibiotics: First-line UTI treatments include nitrofurantoin, trimethoprim-sulfamethoxazole, and fosfomycin. The choice depends on the infection type and bacterial resistance patterns.​

  • Intravenous (IV) antibiotics: Recommended for severe or complicated UTIs such as pyelonephritis. Common IV antibiotics include ceftriaxone, ertapenem, or piperacillin-tazobactam. Vancomycin is used in specific resistant cases, often combined with other agents when gram-positive coverage is needed.

Commonly prescribed medications for UTI

Several antibiotics are commonly prescribed for UTIs:​

  • Nitrofurantoin (Macrobid, Macrodantin): Commonly prescribed for uncomplicated lower UTIs in women. Not effective for kidney infections or in patients with poor kidney function.
  • Trimethoprim/sulfamethoxazole (Bactrim, Septra): A first-line treatment, though resistance may vary by region.
  • Fosfomycin (Monurol): Administered as a single-dose treatment for uncomplicated cases.
  • Cephalexin (Keflex): A cephalosporin antibiotic suitable for various UTIs.
  • Amoxicillin/clavulanate (Augmentin): Used when other antibiotics are unsuitable.
