What is amoxicillin?

Amoxicillin is a widely used antibiotic. It’s an aminopenicillin used to treat bacterial infections. It’s in the group of penicillin medications but has an extra amino group to help combat antibiotic resistance.


Amoxicillin is U.S, Food and Drug Administration (FDA)-approved to treat infections of the ears, nose, and throat, tonsillitis, and pharyngitis in adults and children 12 and older. It’s also approved to treat Helicobacter pylori or H pylori bacteria and duodenal ulcers in adults. Amoxicillin is effective for treating lower respiratory tract infections, bacterial sinusitis, skin infections, and urinary tract infections. 


Amoxicillin is available as a generic medication. Common brand names include Amoxil, Sumox, Trimox, Moxatag, Amoxicot, and Dispermox. Amoxicillin, as the brand name Amoxil, was originally FDA-approved in 1974. When the patent expired on Amoxil, generic versions were approved and became available along with other brand names. 


Off-label uses (something a medication isn’t approved for) for amoxicillin include the treatment of Lyme disease, infectious endocarditis prophylaxis, periodontitis, actinomycosis, and post-exposure prophylaxis to anthrax.

How does amoxicillin work?

Amoxicillin is a class of beta-lactam antibiotics that includes penicillins and cephalosporins. It works by inhibiting penicillin-binding proteins (PBPs). PBPs are enzymes that exist in the walls of your cells and help build and maintain bacterial cell walls. When amoxicillin binds to PBPs, it kills the bacteria. 


Amoxicillin is a broad-spectrum antibiotic. It’s effective in treating a wide range of bacteria, including gram-positive and some gram-negative bacteria like Streptococcus pneumoniae, Haemophilus influenzae, and Escherichia coli. 


Some bacteria though have beta-lactamase enzymes that make amoxicillin inactive. In these cases, amoxicillin is often mixed with clavulanic acid, a beta-lactamase inhibitor. This combination is known as Augmentin (amoxicillin and clavulanate potassium).

Who can take amoxicillin?

Amoxicillin is FDA-approved for use in people of all ages. It’s specifically indicated for use in adults and children with ear infections,  sinusitis, pharyngitis, and infections in the lower respiratory tract, like pneumonia. Your provider may also prescribe amoxicillin for infections of the skin or genitourinary tract. 

In adults only, amoxicillin may be used to treat H pylori or duodenal ulcer disease, sometimes in combination with other medications like clarithromycin and lansoprazole. 

Amoxicillin is sometimes used off-label (not approved for use) for Lyme disease, prevention of bacterial endocarditis during particular medical or dental procedures, and anthrax exposure prophylaxis. 

You shouldn’t take amoxicillin if you have a known allergy to penicillin or beta-lactam antibiotics. If you have severe kidney impairment, your provider may adjust your dose since amoxicillin is mainly excreted by the kidneys.

Amoxicillin dosages and how to use it

Amoxicillin is available as oral capsules, tablets, chewable tablets, and an oral powder for reconstitution. Oral capsules are available in 250- and 500-milligram strengths. Tablets are available in 500- and 875-milligram doses. Chewable tablets are 125- or 250 milligrams. The oral powder for reconstitution is available in 200 milligrams per 5 milliliters, 250 milligrams per 5 milliliters, and 400 milligrams per 5 milliliters strength.  


For children over the age of 3 months who weigh less than 88 pounds, amoxicillin doses start at 25 milligrams per kilogram per day. For children over 3 months and 88 pounds and adults, a provider may prescribe 500 milligrams every 12 hours or 250 milligrams every 8 hours. If an infection is more severe, a provider may start with a higher dose. 


People with renal impairment require adjusted dosages because amoxicillin is excreted by the kidneys, which can make it less effective. 


You can take amoxicillin with food if it irritates your stomach. 


Take a missed dose of amoxicillin as soon as you remember it. But if it’s almost time for your next dose, skip the missed dose. Continue with your regular dosing schedule as prescribed. Don’t double your dose if you miss one. 


Amoxicillin should be stored at room temperature away from heat.

Amoxicillin side effects and warnings

Amoxicillin is generally well tolerated and is an effective antibiotic for treating a variety of infections. 


You may still experience possible side effects. Common side effects of amoxicillin include nausea, vomiting, diarrhea, dizziness, anxiety, insomnia, and mild abdominal pain. These side effects are often temporary and may resolve when your body adjusts to amoxicillin. Taking amoxicillin with food can help. Other reactions may include skin rashes or itching, which may be symptoms of a minor allergic reaction.


Rare but adverse effects include severe allergic reactions such as anaphylaxis, which can be fatal. Symptoms of anaphylaxis include shortness of breath, swelling of the face or throat, and rapid heart rate. If you experience these types of reactions, seek immediate medical attention. 


Amoxicillin may also cause an imbalance of gut bacteria and result in Clostridioides difficile-associated diarrhea, a condition that can become severe. It can also cause serious skin reactions like Stevens-Johnson syndrome, toxic epidermal necrolysis, drug reactions with eosinophilia and systemic symptoms, and acute generalized exanthematous pustulosis. 


Amoxicillin is contraindicated (not recommended for use) in people who have a serious hypersensitivity reaction to amoxicillin or other beta-lactam antibiotics, like penicillins or cephalosporins. 


People with renal impairment can take amoxicillin but may need different doses depending on the severity of their symptoms. Your provider can assess your symptoms and determine the appropriate dose for you. 


To minimize the risk of serious side effects, tell your provider if you take probenecid, oral anticoagulants, allopurinol, oral contraceptives, or other antibacterials. Also, let your provider know if you’re pregnant or nursing. 


Long-term misuse or overuse of antibiotics, such as amoxicillin, causes resistance against antibiotics, making the medication ineffective. Only take amoxicillin as prescribed by your provider.

What does amoxicillin cost?

Amoxicillin isn’t a costly medication. According to drugs.com and with its discount, the price of 15 capsules of 250 milligrams of amoxicillin is $11.49, while the price of 15 capsules of 500 milligrams is $11.08. 

If your provider prescribes the oral powder, it will cost you between $10.34 for 125 milligrams per 5 milliliters and $10.97 for 400 milligrams per 5 milliliters. 

Oral tablets and chewable tablets are more expensive. For 100 oral tablets at 500 milligrams, you pay $22.90. And 100 chewable tablets at 125 milligrams cost $23.76.

How does amoxicillin compare to other antibiotics?

Amoxicillin is effective in treating a range of infections by killing bacteria and preventing further bacterial growth. Amoxicillin isn’t effective in treating beta-lactamase-producing bacteria. Instead, Augmentin, or amoxicillin clavulanate is used. Augmentin is typically used to treat infections caused by resistant bacterial strains that can occur in your ears or sinuses. 


Other common antibiotics include metronidazole, azithromycin (Z-Pak), and cefdinir. Each has different uses than amoxicillin. 


Metronidazole is a common antibiotic that acts on anaerobic, protozoa, and microaerophilic bacterial infections. It’s FDA-approved to treat infections in the bones and joints, brain abscesses, meningitis, endocarditis, endometriosis, bacterial vaginosis, and more. It doesn’t work for respiratory or urinary tract infections like amoxicillin does.


Z-Pak (azithromycin) is a macrolide antibiotic. It’s often used to treat pneumonia and sexually transmitted infections. It’s also approved to treat skin infections, pharyngitis, and upper respiratory infections. It’s commonly prescribed to people allergic to a penicillin antibiotic.


Cefdinier is a second-generation similar to amoxicillin but with even greater resistance to beta-lactamase enzymes. It’s approved to treat complicated urinary tract infections and ventilator-associated pneumonia. It treats a broader range of bacteria than amoxicillin, including some bacteria resistant to amoxicillin. It’s usually prescribed in cases of severe or recurrent infections.


The side effects of the antibiotics mentioned here are similar and include nausea, diarrhea, vomiting, stomach pain, and rash. Severe allergic reactions, liver problems, breathing, and heart rhythm issues are also possible.
