What is glipizide for weight loss or diabetes?

Glipizide was approved by the U.S. Food and Drug Administration (FDA) in 1984 to manage high blood sugar levels in adults with type 2 diabetes mellitus when used in combination with diet and exercise. It comes in 2 forms — immediate-release and extended-release. Both forms are oral tablets. Glipizide extended-release is available as the brand name Glucotrol XL. 

While glipizide’s main use isn’t weight loss, some patients may experience weight loss or weight gain as a side effect. 

Healthcare providers may also prescribe glipizide off-label, meaning for something it’s not FDA-approved for, or for other medical conditions at their discretion.

How does glipizide weight loss work?

Glipizide belongs to a class of drugs known as sulfonylureas. These drugs stimulate the pancreas to produce more insulin, which helps lower blood sugar levels. Weight gain or loss weren’t reported in clinical trials, but both can be a side effect of glipizide.  

Compared to other weight loss meds, glipizide has minimal weight-related effects. While some users may experience slight weight changes, research doesn’t confirm it as a major factor. Instead, studies suggest that lifestyle changes, such as incorporating exercise and adjusting diet, are more likely to contribute to weight loss results when taking glipizide.

Who can take glipizide for weight loss?

Glipizide is FDA-approved for glycemic control, or blood sugar control, in adults with type 2 diabetes to be used along with diet and exercise. Glipizide isn’t an FDA-approved weight-loss drug. People with type 1 diabetes shouldn’t take it.

Glipizide dosages and how to use it

Glipizide is available in immediate-release and extended-release oral tablets. The immediate-release version comes in 5- and 10-milligram tablets, while glipizide ER (extended-release) comes in 2.5, 5, and 10-milligram tablets. For both forms, there’s no standard dosing regimen for people with type 2 diabetes. Instead, dosing is determined by monitoring blood glucose levels and working with your provider.

Glipizide extended-release is also available as brand name Glucotrol XL in the same dosage strengths.

The starting dose of immediate-release glipizide is 5 milligrams taken once a day before breakfast. Some people with type 2 diabetes experience adequate blood sugar control with a once-daily dose. If blood sugar control isn’t achieved, a provider may recommend dividing your dose and/or increasing your dose in increments of 2.5 milligrams to 5 milligrams once a day. The maximum dose you should take in a single day is 15 milligrams if taken all at once. If the dose is split into smaller amounts throughout the day, the total shouldn’t exceed 40 milligrams in a day.

The recommended starting dose of glipizide extended-release (glipizide ER) or Glucotrol XL is 5 milligrams once a day, taken with breakfast. If you don’t get adequate blood sugar control on that dose, your provider may have you adjust your dose up to 10 milligrams a day. The maximum recommended daily dose is 20 milligrams.

People at risk of hypoglycemia (low blood sugar), such as elderly people and those with impaired liver function, may be advised to start with a dose of 2.5 milligrams for either immediate or extended-release glipizide.

Glipizide side effects and warnings

Common side effects of glipizide include asthenia (lack of energy), headache, dizziness, nervousness, tremors, diarrhea, flatulence (gas), and allergic skin reactions.

There’s also the potential for serious side effects on glipizide, such as severe hypoglycemia (low blood sugar), increased cardiovascular mortality compared to treatment with dietary changes alone or dietary changes and insulin use, allergic reactions, and liver problems.

Due to drug interactions, people with type 1 diabetes or those using beta-blockers or nonsteroidal anti-inflammatory drugs (NSAIDs) shouldn’t take glipizide. Individuals with liver or kidney problems, such as kidney disease, or those with diabetic ketoacidosis should use glipizide cautiously. Glipizide may not be recommended for people prone to hypoglycemia. Individuals with glucose-6-phosphate dehydrogenase (g6pd deficiency) should be monitored on glipizide because it can cause hemolytic anemia.

How does glipizide weight loss compare to other weight loss medications?

Glipizide’s weight-related effects are minimal at best. Clinical trials for glipizide haven’t consistently shown significant weight gain or loss as a side effect. A review of different studies shows that while some people taking glipizide experience modest weight fluctuations, there’s no substantial research to confirm it as a major contributor to weight gain. But, there’s also little support for it promoting significant weight loss, although better results were achieved when it was used along with changes in diet and exercise.

Other FDA-approved diabetes medications and weight loss medications, like Wegovy (semaglutide) and Zepbound (tirzepatide), have produced better weight loss results.

Wegovy is an injectable medication that helps control appetite by acting as a glucagon-like peptide-1 (GLP-1) receptor agonist, mimicking a natural hormone. In clinical trials, people using semaglutide in Wegovy experienced weight loss of up to 14.9% of their initial body weight over 68 weeks.

Zepbound, is a dual GLP-1/GIP agonist that targets both GLP-1 and gastric inhibitory polypeptide (GIP), which helps reduce hunger and promote feelings of fullness, leading to lower calorie intake and weight loss. In clinical trials, people using Zepbound lost up to 20.9% of their body weight over 72 weeks.

Ozempic (semaglutide) and Mounjaro (tirzepatide) are similar to Wegovy and Zepbound but FDA-approved to help control blood sugar in people with type 2 diabetes. Both can result in weight loss. With people taking Ozempic losing 5 to 10% of their initial body weight in 40 weeks. And those taking Mounjaro losing up to 13.9% in 104 weeks.

Another FDA-approved weight loss drug is Contrave (naltrexone HCI/bupropion HCI). It works by influencing the brain’s neurotransmitters to help reduce appetite, control cravings, and curb emotional eating. By increasing levels of dopamine and norepinephrine, Contrave can help reduce hunger and lead to weight loss. In clinical trials, this FDA-approved medication led to a weight loss of 5 to 10% of patients’ starting body weight over 56 weeks.

Qsymia (phentermine/topiramate ER) combines phentermine’s appetite-suppressing effects with topiramate’s metabolic-boosting properties. Phentermine increases norepinephrine levels in the brain to reduce hunger, while topiramate can promote feelings of fullness. Together, these ingredients are believed to result in more significant weight loss than phentermine or topiramate alone. In clinical trials, people using Qsymia lost between 5 to 10% of their initial body weight within 56 weeks.

Wegovy, Zepbound, Contrave, and Qsymia are all FDA-approved treatments for weight loss or weight management in individuals with obesity or overweight and 1 weight-related health condition when used alongside a reduced-calorie diet and regular exercise. While these medications can be more effective than glipizide, they can cause side effects, including nausea, vomiting, anxiety, diarrhea, constipation, insomnia, headaches, dry mouth, and increased blood pressure. Ozempic and Mounarjo can also cause these side effects. Qysmia is a controlled substance and has the potential to raise heart rate and blood pressure and has a risk of dependency and misuse.
