Symptoms of mononucleosis

Mononucleosis, or “mono," is a viral illness most often caused by the Epstein-Barr virus (EBV). It typically affects teens and young adults, but can present itself at any age. Symptoms usually appear four to six weeks after exposure, and may include:

  • Extreme fatigue
  • Fever
  • Sore throat (sometimes with white patches)
  • Swollen lymph nodes (neck and armpits)
  • Headache
  • Skin rash (small, reddish patch or spot)
  • Soft, enlarged spleen
  • Loss of appetite
  • Night sweats

Symptoms vary in severity and duration. While many people recover in 2–4 weeks, fatigue can last longer. A licensed provider can confirm your diagnosis and recommend the best course of action for treatment.

How is mononucleosis diagnosed?

Diagnosis typically involves both a clinical evaluation and lab testing. Here’s what you can expect to get tested for by a clinician:

Mononucleosis treatment options

Mononucleosis is a viral infection, so antibiotics aren’t effective. The exception is when there’s a secondary bacterial infection. Most cases improve with supportive care.

For mild to moderate symptoms:

  • Rest: Always critical for immune recovery.
  • Hydration: Fluids help soothe the throat and prevent dehydration.
  • Pain relief: OTC medications, like acetaminophen or ibuprofen, ease fever and sore throat.
  • Throat soothers: Saltwater gargles or lozenges provide local relief.
  • Activity changes: Avoid strenuous activity or contact sports to prevent spleen injury.

    For more serious cases:
  • Corticosteroids: May be prescribed to reduce throat inflammation or breathing difficulty.
  • Antiviral medications: Acyclovir is sometimes used, though it’s generally not effective for uncomplicated mono.
  • Hospitalization: Rare but may be needed for airway obstruction, liver issues, or splenic rupture.

    Secondary bacterial infections, such as strep throat, may require antibiotics like penicillin or azithromycin. Ampicillin or amoxicillin is never prescribed since it can cause rashes in mono patients.

What are the main medications to treat mononucleosis?

While there is no specific antiviral treatment for mononucleosis, some medications can help manage symptoms and prevent complications.

Over-the-counter (OTC) options:
Acetaminophen (Tylenol): Used to reduce fever and relieve pain.
Ibuprofen (Advil, Motrin): Helps alleviate inflammation, fever, and pain.
Throat Lozenges and Sprays: Products containing benzocaine or phenol can soothe sore throats.

Prescription options:
Corticosteroids: Medications like prednisone may be prescribed in severe cases to reduce inflammation of the throat and tonsils, especially when there’s a risk of airway obstruction.
Antibiotics: If a secondary bacterial infection is present, such as streptococcal pharyngitis, antibiotics like penicillin or azithromycin may be prescribed. It’s important to avoid ampicillin or amoxicillin in mono patients, as these can cause a rash.
Antiviral agents: Drugs like acyclovir have been studied, but they have not shown significant benefits in treating uncomplicated mono, and are not commonly used.
