What is gonorrhea?

Gonorrhea is a sexually transmitted infection (STI). It’s caused by the bacterium Neisseria gonorrhoeae and spreads through unprotected sex. Gonorrhea can affect your genitals, rectum, and throat. Many people have no or mild symptoms. But, gonorrhea can lead to painful or uncomfortable symptoms and potentially serious complications. The infection can be treated with antibiotics, but some strains have become resistant. Detecting gonorrhea early is key to determining the best treatment option.

Causes of gonorrhea

Gonorrhea is caused by a bacterium that spreads through unprotected sexual contact. The bacteria thrive in warm, moist areas of the body. So, gonorrhea often affects the genitals, rectum, and throat.

Transmission happens when someone with gonorrhea passes it to you through bodily fluids. It can also be transmitted from parent to baby during childbirth.

Risk factors for gonorrhea include having multiple sexual partners, inconsistent use of condoms, and a history of STIs.

What are the common symptoms of gonorrhea?

Gonorrhea can cause a range of symptoms. Common symptoms include:

  • Painful or burning sensation when urinating
  • Increased vaginal discharge
  • Vaginal bleeding between periods
  • A white, yellow, or green discharge from the penis
  • Painful or swollen testes
  • Rectal discharge
  • Rectal itching or soreness
  • Bleeding from the rectum
  • Painful bowel movements

Some people have mild or no symptoms. Regular STI testing and safe sex practices are recommended to prevent transmission and for early detection. If you notice symptoms or if your partner has symptoms, reach out to your healthcare provider.

How to treat gonorrhea

Treatment for gonorrhea can depend on the severity of the infection and if you have complications.

Mild cases can be treated with antibiotics, a single intramuscular injection of ceftiaxone. In some cases, your provider may also prescribe an additional oral dose of doxycycline or azithromycin if you have another infection.

For people who have antibiotic-resistant strains, your provider will check to see if your symptoms persist after the initial dose. If so, your provider may request testing to determine the best treatment option.

If left untreated, gonorrhea can lead to severe complications like PID, epididymitis, infertility, chronic pain, or an increased risk of other infections. These conditions may require hospitalization and intravenous antibiotics.

Common medications to treat gonorrhea

There are no effective over-the-counter (OTC) medications for the treatment of gonorrhea. Because it’s caused by a bacterium, it requires an antibiotic. The most commonly used medications to treat it include:

  • Ceftriaxone: the primary treatment recommended by health authorities.
  • Azithromycin or Doxycycline: may be prescribed alongside ceftriaxone if you have another infection, like chlamydia.

In the case of antibiotic-resistant strains of the bacterium, providers may prescribe alternative medication options, like gentamicin or spectinomycin.

Taking the full course of antibiotics, as prescribed by your provider, is important to fully clear the infection and prevent antibiotic resistance.

When to see a doctor for gonorrhea treatment

If you experience symptoms like painful urination, discharge, pelvic or testicular pain, or rectal discomfort, contact your healthcare provider as soon as possible.

Gonorrhea can also be asymptomatic, so regular STI testing is recommended if you’re sexually active or have new or multiple partners.

What doctors can treat gonorrhea?

Different types of healthcare providers can treat gonorrhea. Some of them include:

  • Primary care physicians
  • Gynecologists
  • Urologists
  • Infectious disease specialists

You can also visit sexual health clinics or organizations like Planned Parenthood, which offer STI testing and treatment.
