What is sildenafil(generic Viagra)?

Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor that was initially developed by Pfizer as a potential treatment for angina, a condition caused by reduced blood flow to the heart.

Sildenafil was approved by the United States Food and Drug Administration (FDA) in 1998 for the treatment erectile dysfunction (ED). While it is most commonly used for treating ED, sildenafil is also approved under the brand name Revatio to manage pulmonary arterial hypertension (PAH), a condition involving high blood pressure in the arteries connecting the heart and lungs. The FDA approved sildenafil for PAH treatment in adults in 2005 and extended approval to pediatric patients (ages 1-17) in 2023.

Sildenafil is the generic version of many brand names, including Viagra, Aronix, Liberize, Nipatra, Revatio, and Grandipam. It is available in tablet, liquid suspension, and intravenous forms.

How does sildenafil work?

Sildenafil works by relaxing the smooth muscles in blood vessels, increasing blood flow to specific areas of the body. It does this by inhibiting the enzyme phosphodiesterase type 5 (PDE5).

During sexual stimulation, a chemical called nitric oxide is released in the erectile tissue, triggering the production of cyclic guanosine monophosphate (cGMP). cGMP relaxes smooth muscle cells, allowing more blood to flow into the penile tissues, resulting in an erection. But PDE5 breaks down cGMP, preventing blood flow and an erection.

By blocking PDE5, sildenafil prevents the breakdown of cGMP, enhancing and prolonging an erection. It’s important to note that sildenafil is effective only with sexual stimulation and does not induce erections in the absence of arousal.

For pulmonary arterial hypertension, sildenafil relaxes blood vessels in the lungs, lowering pulmonary blood pressure and improving exercise capacity.

Who can take sildenafil?

Sildenafil is FDA-approved for males ages 18 years and older experiencing erectile dysfunction and for adults with pulmonary arterial hypertension. 

Individuals taking nitrates for chest pain, a GC stimulator such as riociguat, or those with severe heart or liver problems, recent history of stroke or heart attack, or low blood pressure should avoid sildenafil. Sildenafil is also not recommended for those with left ventricular outflow obstruction, individuals with severely impaired autonomic control of blood pressure, or those who are taking alpha-blockers. Additionally, according to DailyMed Section 5.1, there are specific precautions for patients who have experienced a heart attack, stroke, or life-threatening arrhythmia within the past six months, as they should take sildenafil with caution and only under careful medical supervision.

How to get a sildenafil online prescription

Getting a sildenafil prescription online involves consulting with a licensed healthcare provider. This can include a medical doctor (MD),  physician assistant (PA), or ​​nurse practitioner (NP). During the consultation, the provider will assess your medical history, current medications, and whether or not sildenafil may be right for you. Sildenafil dosages and how to use it

For erectile dysfunction, sildenafil comes in 20 mg, 25 mg, 50 mg, and 100 mg tablets. The recommended starting dose is 50 mg, taken as needed. Sildenafil should be taken approximately one hour before sexual activity for optimal results but can be taken anywhere from 30 minutes to four hours beforehand. The maximum recommended dose is 100 mg once per day. Sildenafil tablets can be taken with or without food but should be swallowed whole and not crushed or chewed. Sildenafil is also available from BlueChew as a chewable tablet in 30 mg and 45 mg strengths; however, it is important to note that the chewable tablet dosage form and these strengths have not been approved by the FDA for the treatment of erectile dysfunction.

For pulmonary arterial hypertension, Revatio is available in 20 mg oral tablets or as 1.57 grams of sildenafil citrate per bottle, reconstituted to 10 mg/mL. The recommended starting dose is 20 mg three times a day, with a maximum dose of 80 mg three times a day. Each dose should be taken four to six hours apart, with or without food.

The intravenous solution should only be administered by a healthcare professional in a medical facility.

Sildenafil side effects and warnings

Common side effects of sildenafil include headaches, flushing, limb pain, muscle pain (myalgia), back pain, indigestion (dyspepsia), and diarrhea.

Serious side effects, though rare, can include sudden hearing loss, vision loss, low blood pressure (hypotension), vaso-occlusive crisis in patients with pulmonary hypertension secondary to sickle cell disease, and priapism—a prolonged, painful erection that requires immediate medical attention.

Sildenafil can interact with certain medications, including nitrates, riociguat, CYP3A inhibitors, and other phosphodiesterase-5 (PDE5) inhibitors, potentially leading to dangerous drops in blood pressure.  Additionally, sildenafil may interact specifically with amlodipine and alpha-blockers such as doxazosin, as noted in the Drug Interactions and Section 7 of DailyMed. Individuals with cardiovascular conditions, severe liver impairment, or retinitis pigmentosa should use sildenafil with caution and under medical supervision.

How does sildenafil compare to other ED medications?

Sildenafil (Viagra) is a well-known phosphodiesterase type 5 (PDE5) inhibitor used primarily for erectile dysfunction (ED), but other medications in this class are also available.

Tadalafil (Cialis) is approved for ED and benign prostatic hyperplasia (BPH). It is known for its longer duration of action, lasting up to 36 hours—significantly longer than sildenafil’s 4 to 6 hours. This allows for more spontaneity in sexual activity. However, it may take longer to start working (30 minutes to 2 hours), and some users report back or muscle pain as a common side effect.

Vardenafil (Levitra), like sildenafil, is FDA-approved for ED and works by enhancing blood flow to the penis. Its onset is quick, typically within 30 to 60 minutes, and its effects last around 4 to 6 hours. A slight advantage over sildenafil is that it absorbs better when taken with food, whereas sildenafil may be less effective after a heavy meal. However, vardenafil may cause more headaches and flushing than sildenafil.

Avanafil (Stendra) is a newer PDE5 inhibitor that works similarly to sildenafil but has a faster onset of action—within 15 to 30 minutes. Its duration of effect is also around 4 to 6 hours, and it tends to have fewer side effects, such as flushing or headaches. However, it may be more expensive, and its long-term safety profile is not as well-established as sildenafil’s.
