I made a switch from a traditional Doctor's office visits (one of my appointments took 2 hours before my doctor could see me) to Elevate Wellness Direct Primary Care and I am so glad I did! My Care Provider, Sarah Perales FNP-C is thorough with my needs and scheduled me for an appointment immediately after I signed up. She took time with me carefully going over my family's history + what my health goals are to get me the right health plan going forward. Sarah was genuinely concerned about my health needs and methodically went over my plan of care. Anytime I have questions she responds in a timely manner. I'm so glad we met and I am on a good health plan that fits my needs. Sarah is competent, respectful, kind and sensitive to my needs.
