Cigna commercial plans in New York may cover some obesity evaluation, nutrition counseling, and weight-loss medications. Coverage typically depends on the medical policy, Express Scripts formulary, prior authorization, BMI criteria, and whether the drug is for obesity versus another labeled use. New York mental-health parity (Timothy’s Law) does not require GLP-1 coverage. Always verify your own benefits before you book.

Quick answer for 2026

Many Cigna members in New York can use in-network telehealth for medically appropriate obesity care when the plan lists the visit as a covered service. Medication coverage is separate. Express Scripts (ESI) typically manages the pharmacy benefit. Unlike some CVS/Caremark lists that dropped Zepbound on July 1, 2025 and later reinstated it (October 1, 2026 on many lists), Cigna/ESI historically did not remove Zepbound in the same way; many ESI lists still place GLP-1s on higher tiers with PA. That is plan-specific, not a guarantee.

Cigna PA policy IP0477 (effective May 15, 2026) requires prior authorization for stimulants. It does not govern GLP-1s, Contrave, or orlistat. Phentermine and Qsymia are Schedule IV; GLP-1s, Contrave, and orlistat are not scheduled.

New York rules that apply (and ones that do not)

  • Federal MHPAEA and N.Y. Ins. Law §§ 3221 and 4303 (Timothy’s Law) address mental health and substance use parity. They do not create a statewide mandate that commercial plans must cover anti-obesity GLP-1s.
  • N.Y. Ins. Law §§ 3217-h and 4306-g (and Public Health Law § 4406-g) address telehealth delivery for many state-regulated plans. DFS FAQs: https://www.dfs.ny.gov/apps-and-licensing/health-insurers/telehealth-ins-prov-info.
  • Self-funded ERISA plans follow federal rules and the employer SPD, not DFS complaint jurisdiction in the same way.
  • HHS/DEA telemedicine flexibilities for many controlled-substance prescribing currently run through December 31, 2026. Most first-line GLP-1s are not controlled.

How Express Scripts usually treats obesity drugs

Cigna members typically check the 2026 drug list on Cigna’s prescription drug lists and the member portal on Express Scripts. Plans often:

  • Place Wegovy, Zepbound, Saxenda, and similar agents on Tier 3-4 with PA, step therapy, and quantity limits
  • Require documented BMI, comorbidity, and lifestyle program notes
  • Cover metformin, Contrave, or orlistat on different tiers when medically necessary
  • Exclude compounding or cash-pay “med spa” products

Medicare GLP-1 Bridge (if you have Part D, not commercial Cigna)

If you have Medicare Part D (including many Medicare Advantage pharmacy benefits), CMS launched a Medicare GLP-1 Bridge starting July 1, 2026. Eligible beneficiaries may pay about $50 per month for certain products (Wegovy, Zepbound, Foundayo) through December 31, 2027, outside ordinary commercial ESI rules.
