Does Cigna Cover Weight Loss Treatment in Florida? A 2026 Guide

Cigna commercial plans in Florida may cover medically necessary obesity evaluation, follow-up, and some weight-loss medications when the visit is in network and allowed on your medical policy and Express Scripts list. Coverage is not automatic. Federal MHPAEA and Fla. Stat. § 627.668 set a floor for many fully insured mental-health benefits. They do not force every GLP-1, phentermine, Contrave, or orlistat product onto a preferred tier. Always verify your own benefits before you book.

TLDR

  • Cigna may cover medically necessary weight-management visits and some anti-obesity drugs in Florida. Benefits vary by employer PPO/HMO, Florida Marketplace, Medicare Advantage, and Medicaid managed care products.
  • Federal MHPAEA plus Fla. Stat. § 627.668 (optional/make-available mental health coverage on many fully insured policies) apply to many commercial products. They do not rewrite self-funded ERISA plan documents or Medicare Advantage formularies. § 627.668 is a mental-health make-available rule, not a GLP-1 mandate.
  • Florida Statutes § 456.47 governs telehealth practice. It does not force Cigna to pay every video visit. Coverage still depends on the plan.
  • Pharmacy benefits typically run through Express Scripts (ESI). Cigna/ESI did not remove Zepbound the same way CVS/Caremark did on many Aetna lists. On many Cigna lists Zepbound has remained available, often Tier 3-4, with PA and step therapy. That still does not guarantee payment for your NDC.
  • Cigna PA Policy IP0477 (effective May 15, 2026 on many Cigna commercial lists) typically requires prior authorization for stimulants. It is an ADHD/stimulant PA policy. Do not treat it as a GLP-1 PA rule.
  • Phentermine and Qsymia are Schedule IV. GLP-1s (Wegovy, Zepbound, Saxenda), Contrave, and orlistat are typically unscheduled. HHS/DEA telemedicine flexibilities for prescribing many controlled medications currently run through December 31, 2026.
  • CVS/Caremark Zepbound timing (July 1, 2025 removal / October 1, 2026 reinstatement on many lists) is an Aetna/Caremark story. It does not control Express Scripts obesity drugs.
  • Medicare GLP-1 Bridge ($50/month Wegovy/Zepbound/Foundayo via CMS, starting July 1, 2026) is a Medicare obesity program. It does not pay for commercial Cigna GLP-1s.
  • Verify benefits on myCigna and Express Scripts. For many fully insured Florida complaints, Florida DFS Division of Consumer Services / FLOIR consumer help is often 1-877-693-5236 (1-877-MY-FL-CFO). Medicare Advantage appeals follow CMS, then often Maximus IRE at 1-855-887-6668. AHCA/Medicaid products follow Florida Medicaid rules, not this commercial page. Does Cigna cover weight loss treatment in Florida?

Often, some of it, with caveats. Evaluation visits and medication management may process as medical benefits on many Cigna commercial plans when the visit is medically necessary and in network. Medication is a separate pharmacy question. Copays, deductibles, BMI criteria, step therapy, and whether Klarity is in network still sit in your Summary of Benefits and Coverage, medical policy, and Express Scripts drug list.

Self-funded ERISA employer plans follow federal rules and the plan document more than Fla. Stat. § 627.668. Medicare Advantage, Medicaid, and dual-eligible products follow CMS and Florida Medicaid rules, not § 627.668 in the same way. Always treat this page as education, not a coverage determination.

How common is adult obesity?

According to NCHS Data Brief No. 508, the prevalence of obesity among U.S. adults was 40.3% during August 2021–August 2023, with no significant differences between men and women. Severe obesity prevalence was 9.4% and was higher in women than men for each age group. From 2013-2014 through August 2021–August 2023, age-adjusted obesity did not change significantly, while severe obesity increased from 7.7% to 9.7%.

Those figures describe national epidemiology. They do not decide whether your Cigna Florida plan pays for a video visit or a brand-name GLP-1.

Parity in Florida: what § 627.668 does and does not do

Two layers matter in 2026:

  1. Federal MHPAEA. Group health plans and many individual plans that cover mental health generally cannot apply more restrictive financial requirements or treatment limitations than they apply to medical/surgical care in the same classification. Obesity pharmacotherapy is usually a medical or pharmacy benefit, not a mental-health benefit, so MHPAEA does not by itself force GLP-1 coverage.
  2. Fla. Stat. § 627.668 addresses optional/make-available coverage for mental and nervous disorders on many fully insured policies written in Florida. See the statute text on Online Sunshine. It is a make-available mental health rule, not a mandate that Wegovy, Zepbound, phentermine, or Contrave must sit on a preferred tier.

Parity does not mean every anti-obesity drug is preferred or that prior authorization is illegal. If Cigna denies a brand GLP-1 after a generic or lifestyle-program step, you still use the plan’s appeal path.

Express Scripts, Zepbound, and what IP0477 is not

Most Cigna commercial pharmacy benefits in Florida run through Express Scripts. Check the 2026 drug lists on Cigna’s prescription drug lists and your member portal at Express Scripts.

On many Cigna/ESI lists, Zepbound was not removed the way CVS/Caremark removed it from many Aetna lists on July 1, 2025 (with reinstatement on many Caremark lists October 1, 2026). Zepbound on Cigna has often remained on higher tiers (commonly described as Tier 3-4) with PA. Wegovy, Saxenda, Contrave, orlistat, phentermine, and Qsymia each have their own PA, quantity-limit, and BMI rules. Confirm the exact NDC on your card.

IP0477 is Cigna’s stimulant prior-authorization policy. It typically applies to ADHD stimulants. It does not rewrite obesity PA criteria.

Telehealth, scheduling, and Florida practice rules

Fla. Stat. § 456.47 sets practice standards for telehealth in Florida. It does not require Cigna to reimburse every video visit at in-network rates. HHS/DEA telemedicine flexibilities for many controlled medications currently run through December 31, 2026. Phentermine and Qsymia (Schedule IV) sit inside that controlled-substance conversation. GLP-1s, Contrave, and orlistat typically do not.

Klarity clinicians still follow DEA, Florida Board, and plan rules. A video visit does not by itself create a covered GLP-1 fill.

Medicare, Medicaid, and the GLP-1 Bridge

Medicare Advantage Cigna products follow CMS 42 CFR Part 422, not Fla. Stat. § 627.668. The Medicare GLP-1 Bridge ($50/month Wegovy/Zepbound/Foundayo via CMS from July 1, 2026) is a Medicare obesity program. It does not apply to commercial Cigna ESI fills.

Florida Medicaid and AHCA managed-care products follow Medicaid formulary and PA rules. This commercial page does not describe those products. Dual-eligible members should confirm which card pays first.

How to verify Cigna weight-loss benefits in Florida

  1. Log into myCigna and Express Scripts. Search Wegovy, Zepbound, Saxenda, phentermine, Contrave, and orlistat by NDC if you have one.
  2. Ask whether obesity pharmacotherapy is a covered class, excluded lifestyle benefit, or PA-only class, and what BMI or comorbidity criteria apply.
  3. Confirm whether Klarity or your clinician is in network for the medical visit, separate from the pharmacy benefit.
  4. If the plan is fully insured in Florida and you cannot resolve a denial, Florida DFS Division of Consumer Services often uses 1-877-693-5236. See DFS Consumer Services contact.
  5. Medicare Advantage denials typically go through the plan, then CMS redetermination, then often Maximus IRE at 1-855-887-6668.

FAQ

Does Cigna cover Wegovy or Zepbound in Florida?

Sometimes, when the plan includes anti-obesity drugs, PA is approved, and the NDC is on the Express Scripts list. Cigna/ESI generally did not follow the Caremark 2025 Zepbound removal. Coverage still varies. Verify the current list.

Does Fla. Stat. § 627.668 require Cigna to cover GLP-1s?

No. That statute is about make-available mental-health coverage on many fully insured policies. It is not a GLP-1 mandate.

Is phentermine covered on Cigna in Florida?

Phentermine is Schedule IV. Some Cigna commercial lists include it with PA or quantity limits. Coverage is not guaranteed. Confirm with Express Scripts.

Does IP0477 block weight-loss drugs?
