Anthem commercial plans in Florida may include evaluation, nutrition counseling, and some prescription weight-loss options, but obesity drugs are often carved out, tiered, or gated by prior authorization. Coverage varies by employer group, marketplace metal level, and pharmacy benefit. Fla. Stat. § 627.668 is a make-available mental health statute; it does not force GLP-1 or phentermine coverage. Florida Blue is a separate company. Always verify your ID card, formulary, and medical policy before you book.

Does Anthem cover weight loss treatment in Florida?

Often, parts of it. Evaluation with a licensed clinician, follow-up, and related labs may process as medical benefits when the visit is medically necessary. Anti-obesity medications sit on the pharmacy benefit. Many groups treat obesity drugs as excluded, limited, or available only after documented diet and activity programs. MHPAEA and Fla. Stat. § 627.668 do not convert an obesity exclusion into a mandate.

CDC NCHS data for August 2021–August 2023 put U.S. adult obesity at 40.3% (NCHS Data Brief 508). Demand is high; plan language still decides payment.

Anthem vs Florida Blue

Anthem Blue Cross and Blue Shield in Florida is not Florida Blue (Blue Cross and Blue Shield of Florida). Networks, PBMs, and appeal addresses differ. Read the carrier name on the card, not only the Blue Cross mark.

Carelon Rx and typical 2026 pharmacy rules

Many Anthem commercial groups use Carelon Rx. Expect:

  • BMI thresholds (often 30, or 27 with a comorbidity) in the medical policy, not on the ID card.
  • Step therapy through older agents before a branded GLP-1.
  • Quantity limits and specialty-pharmacy routing for injectables.
  • Phentermine and Qsymia as Schedule IV; GLP-1s, Contrave, and orlistat are typically unscheduled.

Do not apply Aetna/CVS Caremark Zepbound removal (July 1, 2025) and reinstatement (October 1, 2026) to Anthem. Do not apply Cigna IP0477 stimulant PA to obesity drugs.

Medicare GLP-1 Bridge vs commercial Anthem

If you have Humana or another Medicare Advantage plan, CMS’s GLP-1 Bridge (about $50/month for specified products from July 1, 2026) may apply. That is federal Medicare policy, not Carelon Rx. Anthem commercial members should not assume that $50 copay.

Telehealth in Florida

Fla. Stat. § 456.47 sets telehealth practice standards. Coverage still follows your medical and pharmacy policies. HHS and DEA extended controlled-substance telemedicine flexibilities through December 31, 2026 (HHS/DEA announcement). Phentermine and Qsymia remain Schedule IV; a clinician still must be licensed and DEA-registered as required.

Will Anthem pay for Wegovy or Zepbound in Florida?

It may, if the drug is on your Carelon list and you meet policy criteria. Many groups exclude or tightly manage GLP-1s for weight loss. Confirm the current formulary.

Does § 627.668 require obesity drug coverage?

No. It addresses optional mental and nervous disorder coverage that group insurers must make available. It is not a GLP-1 mandate.
