Anthem commercial plans in Florida may cover ADHD evaluation, follow-up, and some stimulant or non-stimulant medications when the visit is medically necessary, in network, and allowed on your medical policy and Carelon Rx list. Coverage is not automatic. Federal MHPAEA and Florida Statutes § 627.668 set a floor for many fully insured group products. Always verify your own benefits before you book.

Disclaimer: This article is educational. It is not a benefits determination, diagnosis, or guarantee of payment. Coverage varies by plan. Verify benefits with Anthem and your clinician before you book. Klarity is not Anthem and does not process claims for Anthem.
Often, some of it, with caveats. Evaluation visits and medication management may process as medical benefits on many Anthem commercial plans when the visit is medically necessary and in network. Medication is a separate pharmacy question. Copays, deductibles, and whether Klarity is in network still sit in your Summary of Benefits and Coverage, medical policy, and Prescription Drug List.
Self-funded ERISA employer plans follow federal rules and the plan document more than Florida Insurance Code. Medicare Advantage, Florida Medicaid, and dual-eligible products follow CMS and AHCA, not § 627.668 in the same way. Always treat this page as education, not a coverage determination.
Do not confuse Anthem with Florida Blue (Blue Cross and Blue Shield of Florida), which is a separate Blue plan. Confirm the legal name on your card.

Two layers matter in 2026:
If Anthem denies a brand stimulant after a generic alternative exists, that is often a formulary or PA issue, not an automatic parity violation. If the denial is a blanket mental-health visit cap that medical visits do not face, parity may apply. Keep those buckets separate when you appeal.

Florida Statutes § 456.47 sets practice standards for telehealth, including out-of-state registration in some cases. It is a licensing statute, not an Anthem payment guarantee. Many fully insured plans still reimburse video visits that would be covered in person when clinically appropriate. Self-funded employers may still pay for telehealth, but Florida insurance code may not bind them.
Klarity providers licensed in Florida can evaluate ADHD by video when clinically appropriate. Same-day appointments are often available. Coverage still depends on network status and cost share.
Federal controlled-substance telemedicine rules remain a separate overlay. HHS and DEA have extended certain telemedicine flexibilities through December 31, 2026. Stimulants remain Schedule II. Atomoxetine, viloxazine, and guanfacine ER are typically not Schedule II. Your clinician still must follow DEA, Florida prescriber, EPCS, and plan rules.

Klarity clinicians can help you understand what a PA packet often includes. They cannot promise Anthem will approve a drug. Do not confuse Carelon Rx with Express Scripts IP0477 (Cigna) or Humana Pharmacy Solutions (typical Medicare Advantage).

A licensed clinician usually reviews history, current symptoms, function at work or school, medical history, and screening for anxiety, depression, sleep problems, and substance use. Rating scales may be part of the visit. A prescription is not automatic. If a stimulant is considered, the clinician still has to meet DEA and Florida prescribing rules.
Klarity’s network includes 2,000+ licensed providers. Availability in Florida depends on licensure and your plan’s directory.
For fully insured commercial denials, start with Anthem’s internal appeal, then Florida DFS if the product is state-regulated. Self-funded ERISA plans use the plan’s appeal and, in many cases, federal external review.
Medicare Advantage follows CMS organization determinations, reconsideration, then often Maximus IRE at 1-855-887-6668. Fla. Stat. § 627.668 does not rewrite MA. Medicaid (including many AHCA managed-care products) follows Florida Medicaid rules. This page is not a Medicaid coverage guide.
