SafePrescriber: Controlled Substance Documentation for Telehealth (2026)

Last updated: October 12, 2026

Independent PMHNPs and psychiatrists who prescribe stimulants, benzodiazepines, or other controlled medications on video visits carry two jobs at once: good clinical care and a durable paper trail. SafePrescriber is Klarity’s patient safety program for controlled substance prescribers. This guide maps the documentation habits telehealth practices need, then points to the live SafePractice toolkit without inventing regulatory outcomes.

TLDR

  • DEA extended COVID-era telemedicine flexibilities for prescribing controlled medications through December 31, 2026 (DEA press release, Dec 31, 2025). Permanent special-registration frameworks may still change; watch DEA/HHS notices.
  • Flexibilities do not replace state license, scope, PMP, and EPCS rules where the patient sits.
  • Klarity SafePrescriber is a commitment program with standards on PMPs, noncompliance response, prescription justification, high-risk meds, monitoring, early refills, frequency, patient education, CME, and informed consent (SafePractice).
  • Klarity EHR lists e-prescribing, documentation, charts, and Zoom telehealth in a free HIPAA-compliant stack available in 50 states (Klarity EHR).
  • Klarity Health, Inc. does not provide medical services; independent practices deliver care on the technology platform.

Who this is for

  • PMHNPs and psychiatrists managing ADHD, anxiety, sleep, or other panels that may include Schedule II–IV meds
  • New independent practices standing up telehealth controlled-Rx workflows
  • Employed clinicians adding a side panel who need consistent chart language

If you never prescribe controlled substances, skim the compliance culture notes and focus on free EHR and no-show protection instead once live.

Federal telemedicine context (2026)

On December 31, 2025, DEA announced another extension of telemedicine flexibilities so DEA-registered practitioners can continue certain controlled-substance prescribing via telemedicine without a prior in-person evaluation, through December 31, 2026 (DEA). Earlier DEA materials also describe longer-term telemedicine registration concepts (DEA Jan 16, 2025 rules announcement).

Treat federal flexibility as permission structure, not a documentation free pass. Boards and payers still expect medical necessity, identity verification, and state PMP use.

What SafePrescriber is on Klarity

SafePractice introduces SafePrescriber as a patient safety program for controlled substance prescribers, built with compliance and quality professionals. Providers who commit pledge integrity, safety, and quality in controlled prescribing.

Public commitments on the page include:

  • Patient safety first: documentation, thorough evaluation, justified prescribing
  • Integrity: refuse fraudulent or careless prescribing; guard against diversion and drug-seeking patterns
  • Thoughtful care: assessment, transparent options
  • Compliance: best practices, legal requirements, PMP collaboration

Klarity states it will not prescribe exactly how each clinic implements standards. Providers review toolkit contents and adopt the same or similar policies (SafePractice). Questions: compliance@helloklarity.com.

SafePrescriber toolkit standards (public list)

The live page links PDFs for ten standards. Use them as your internal checklist; open each PDF from the site for full text:

  1. Using prescription monitoring programs (PMPs)
  2. Response to patient noncompliance
  3. Prescription justification and documentation
  4. Policy on high-risk medications
  5. Patient monitoring and follow-up
  6. Handling requests for early refills
  7. Frequency of prescribing controlled substances
  8. Education on controlled substance risk
  9. CME on prescribing controlled substances
  10. Documentation of patient informed consent

Documentation habits that survive an audit

Before the first controlled Rx (telehealth)

  • Verify identity and location (patient state drives your license and PMP)
  • Complete clinical assessment appropriate to the condition
  • Check the state PMP (and multi-state views when required)
  • Document risk discussion and informed consent
  • Confirm DEA registration and any state controlled-substance permits

In the note

  • Diagnosis and medical necessity in plain clinical language
  • Why this molecule and schedule vs alternatives
  • Dose, quantity, days supply, and refill plan
  • PMP findings (or “checked, no concerning fills”) with date
  • Follow-up interval and monitoring plan (vitals, scales, urine when indicated)
  • Patient education on storage, diversion, and when to call

After the visit

  • e-Prescribe through a controlled-capable workflow
  • Schedule follow-up before the script runs out when clinically required
  • Log early-refill requests and your decision trail (SafePrescriber Standard 6)

Klarity EHR lists e-prescribing, documentation, charts, intake, and telehealth so the note and the Rx live in one system (Klarity EHR). Britannie Bell’s EHR-page testimonial emphasizes reaching patients who struggle to access care (Klarity EHR).

State PMP and multi-state panels

PMP names differ (CURES in California, NJPMP in New Jersey, and so on). Multi-state telehealth means you may need enrollments and queries in every state where patients sit. SafePrescriber Standard 1 centers PMP use; do not treat a single-state check as enough for a multi-state panel.

Early refills, noncompliance, and high-risk meds

Telehealth increases convenience and, for some patients, pressure for early fills. Build a written response path before the first angry message:

  • What counts as lost/stolen vs pattern
  • When you require a police report or in-person step
  • When you taper, refuse, or refer

SafePrescriber publishes dedicated standards on noncompliance, high-risk medications, early refills, and prescribing frequency (SafePractice). Report suspected fraud, waste, or abuse on the platform via Klarity’s linked reporting form on that page.

Ops stack around safe prescribing

Controlled-Rx quality fails when the calendar is chaos. Pair safety standards with:

Join-us volume framing (20+ / 100+ / ~48 hours / 1000+ practices) assumes you can absorb demand without cutting documentation corners (join Klarity).

Implementation path on Klarity

  1. Read SafePractice / SafePrescriber and download toolkit PDFs you will adopt.
  2. Request early access via the forms linked on that page if your onboarding requires it.
  3. List on Klarity and run visits in Klarity EHR (free to start, HIPAA-compliant, 50 states).
  4. Keep CAQH and payer packets current while panels load.
  5. Email compliance@helloklarity.com for program questions; joinklarity@helloklarity.com for joining.
