Symptoms of a yeast infection

Recognizing symptoms early can help prevent complications. Common signs of a vaginal yeast infection include:

  • Vaginal and vulvar itching or irritation
  • Thick, white discharge resembling cottage cheese
  • Redness or swelling of the vulva
  • Burning during urination or sex
  • Vaginal soreness or discomfort
  • Cracked skin around the vulva (in severe cases)
  • Mild odor (yeast infections typically lack a strong smell, unlike bacterial infections)

How is a yeast infection diagnosed?

A licensed provider may use the following methods to confirm a yeast infection and rule out similar conditions:

  • Symptom review: A provider will ask about common symptoms such as itching, burning, thick discharge, redness, or discomfort during urination or sex.
  • Physical examination: A pelvic exam may be performed to inspect the vaginal area for signs of inflammation, swelling, or discharge.
  • Microscopic testing: A sample of vaginal discharge may be examined under a microscope to check for Candida yeast cells.
  • pH testing: Vaginal pH may be measured to help rule out other infections like bacterial vaginosis (yeast infections typically do not alter pH).
  • Culture or lab testing: In recurrent or unclear cases, the discharge may be sent to a lab for fungal culture or DNA testing to identify the specific Candida species.

These steps help confirm whether the symptoms are due to a yeast infection or another condition with similar symptoms, such as bacterial vaginosis or trichomoniasis.

Yeast infection treatment options

Yeast infection treatments depend on the severity and frequency of symptoms.

Mild to moderate infections:

  • OTC antifungals, such as Miconazole or clotrimazole, may be used intravaginally for 1–7 days
  • Oral fluconazole (Diflucan): Single 150 mg dose for uncomplicated VVC

Severe or recurrent infections (4+ per year):

  • Initial therapy: Fluconazole 150 mg every 72 hours for 3 doses (Day 1, 4, 7)
  • Maintenance therapy: Fluconazole 150 mg orally once weekly for 6 months (CDC-recommended, but off-label)
  • Prescription topical antifungals:
    • Butoconazole 2% cream (Gynazole-1): Single-dose intravaginal cream approved for uncomplicated VVC.
    • Terconazole 0.4% cream (Terazol 7): Once daily for seven days, approved for vulvovaginal candidiasis.

Non-albicans Candida infections:

  • May require longer or alternative antifungal regimens

Always consult a licensed provider to determine the best course of treatment based on your symptoms and history.

  • Moderate to severe symptoms
  • Recurrent infections (≥4/year)
  • Failure of OTC treatments

Oral antifungal medications

  • Fluconazole (Diflucan)

    • Recurrent VVC: 150 mg every 72 hours × 3 doses, then weekly for 6 months (off-label)
    • Avoid during the first trimester of pregnancy

Prescription-only topical antifungals

  • Butoconazole (Gynazole-1)
    • Single-dose vaginal cream for uncomplicated VVC
