Does Humana Cover Anxiety Treatment in Texas? A 2026 Guide

Humana coverage for Texas residents is typically a Medicare Advantage (MA) product, not a large fully insured commercial book. Those MA plans may cover medically necessary anxiety evaluation, therapy, medication management, and some anti-anxiety or antidepressant medications when CMS medical-necessity rules, the Humana Pharmacy Solutions (HPS) formulary, prior authorization, and network rules line up. Coverage varies by contract. Always verify your own Evidence of Coverage and Part D list before you book.

TLDR

  • Humana in Texas is typically MA only (commercial exit in many markets). Benefits follow CMS 42 CFR Part 422 and your plan documents, not Texas Insurance Code Chapter 1355 in the same way fully insured commercial policies do.

  • Texas Insurance Code Chapter 1455 addresses telemedicine on many fully insured plans. Medicare telehealth follows CMS. Self-funded ERISA employer plans follow the plan document.
  • Pharmacy is typically Humana Pharmacy Solutions (HPS), not Express Scripts and not CVS/Caremark. Stimulant PA on many Humana lists is an HPS/Part D rule. It is not Cigna PA Policy IP0477 (that policy is Cigna/ESI, ADHD stimulants, effective May 15, 2026).

  • Many SSRIs and SNRIs used for anxiety are typically unscheduled. Benzodiazepines are Schedule IV. HHS and DEA extended certain telemedicine flexibilities for prescribing many controlled medications through December 31, 2026. See the Federal Register extension and DEA announcement.
  • CVS/Caremark dropped Zepbound on many lists on July 1, 2025 and later reinstated it on October 1, 2026. That timeline belongs to Caremark (common on Aetna obesity coverage), not to Humana HPS anxiety benefits.
  • The Medicare GLP-1 Bridge (~$50/month for certain Wegovy, Zepbound KwikPen, and Foundayo products) is a CMS obesity program, not an anxiety benefit. See CMS GLP-1 Bridge.
  • Texas Medicaid (STAR) and dual-eligible products follow Texas Medicaid and CMS, not this MA commercial-style write-up. Klarity does not treat this page as a Medicaid benefit explanation.

Does Humana cover anxiety treatment in Texas?

Often, some of it, with MA caveats. Evaluation visits, therapy, and medication management may process as medical or behavioral-health benefits on many Humana MA plans when the visit is medically necessary and in network. Medication coverage is a separate Part D / HPS question. Copays, deductibles, quantity limits, step therapy, and whether Klarity (or any telehealth group) is in network still sit in your Evidence of Coverage, medical policy, and PDL.

Humana MA versus commercial Cigna or Aetna in Texas

Do not copy a Cigna Texas anxiety guide onto a Humana card:

  1. Humana. Typically Medicare Advantage in Texas. PBM: Humana Pharmacy Solutions. Appeals: CMS organization determination → reconsideration → Maximus IRE.
  2. Cigna. Typically commercial. PBM: Express Scripts. Stimulant PA often cites IP0477. That policy does not govern SSRIs for anxiety and does not govern Humana.
  3. Aetna. Typically commercial. PBM: CVS/Caremark. Zepbound July 2025 / October 2026 timeline is Caremark obesity, not anxiety and not HPS.

Parity: MHPAEA, Chapter 1355, and CMS 42 CFR Part 422

Three layers show up in Texas anxiety searches:

  1. Federal MHPAEA. Group health plans and many individual plans that cover mental health or substance use disorder care generally cannot apply more restrictive financial requirements or treatment limitations than they apply to medical/surgical care in the same classification.
  2. Texas Insurance Code Chapter 1355. Adds state mental health benefit rules for many fully insured policies written in Texas. TDI, not CMS, is the usual complaint desk for those policies.
  3. CMS 42 CFR Part 422. Medicare Advantage organization and coverage rules. MHPAEA and Chapter 1355 do not rewrite an MA Evidence of Coverage the way they rewrite a fully insured PPO.

None of these laws guarantee that Humana must pay a specific SSRI, SNRI, buspirone product, or benzodiazepine, or that a specific telehealth group is in network.

Telehealth: Chapter 1455 versus Medicare telehealth

Chapter 1455 addresses telemedicine and telehealth coverage on many fully insured Texas plans. MA telehealth follows CMS original Medicare telehealth rules as adapted in the MA plan. A video anxiety visit that is payable on a Cigna PPO can still deny on Humana MA if the code, originating site, or provider enrollment does not match CMS and the Humana network file.

HHS and DEA extended certain controlled-substance telemedicine flexibilities through December 31, 2026. That federal clock is separate from Chapter 1455 and from whether HPS places a benzodiazepine on PA.

HPS formulary, SSRIs, and benzodiazepines

Anxiety medication is usually a pharmacy benefit. On Humana MA that means Part D through HPS. Check the current Humana drug list for your contract year. Typical patterns (always confirm on your list):

  • Many generic SSRIs and SNRIs used for GAD, panic, and social anxiety are often preferred generics. Step therapy and quantity limits still appear.
  • Buspirone is typically unscheduled.
  • Benzodiazepines (alprazolam, clonazepam, lorazepam, diazepam) are Schedule IV. PA, quantity limits, and concurrent-use edits are common. Texas PDMP review may apply when a clinician prescribes them.
  • Hydroxyzine is typically unscheduled; coverage still depends on the PDL.

Cigna IP0477 is a stimulant ADHD prior-authorization policy. Do not treat it as Humana’s anxiety PA rule. Caremark’s Zepbound timeline is obesity, not anxiety.

How to check your Humana Texas anxiety benefits

  1. Read the front of your card: Humana MA versus a commercial or Medicaid product.
  2. Open your Evidence of Coverage and the current HPS / Part D formulary for your plan name and year.
  3. Call the number on the card and ask whether outpatient psychiatry, therapy, and telehealth anxiety visits are in network for the specific group you plan to use.
  4. Ask whether your target medication is preferred, needs PA, or has quantity limits.
  5. If Klarity (or another telehealth group) is out of network, ask cash-pay and superbills options. Out-of-network MA rules are stricter than many commercial PPOs.

Appeals: TDI versus Maximus IRE

Fully insured Texas commercial disputes often go to the Texas Department of Insurance Help Line at 800-252-3439. Medicare Advantage organization determinations follow CMS: plan determination, reconsideration, then independent review that often routes through Maximus IRE at 1-855-887-6668. TDI generally does not rewrite an MA denial. Dual-eligible and Medicaid STAR members use Texas Medicaid fair-hearing paths, not this MA checklist.

No. IP0477 is a Cigna/Express Scripts stimulant policy. Humana uses HPS and CMS Part D rules.

Does Chapter 1455 force Humana to pay a video visit?
