Cigna commercial plans in Texas may cover medically necessary depression evaluation, therapy, and medication when the visit is in network, the plan documents list the benefit, and any prior authorization rules are met. Coverage is not automatic. Self-funded employer plans, Medicare Advantage, and Texas Medicaid STAR follow different rules than a fully insured Cigna PPO or HMO sold in Texas.

Often, yes, for a fully insured Cigna commercial plan, when a licensed clinician documents major depressive disorder or a related diagnosis and the care is medically necessary. “Cover” still means copays, deductibles, coinsurance, network rules, and sometimes step therapy. A denial on one medication does not mean the plan refuses all depression care.

The federal Mental Health Parity and Addiction Equity Act generally requires financial requirements and treatment limitations for mental health to be no more restrictive than those for medical/surgical benefits in the same classification. Cigna pharmacy benefits in Texas typically run through Express Scripts. Cigna did not remove Zepbound from ESI commercial formularies the way CVS/Caremark did in 2025. That obesity timeline is irrelevant to depression SSRI/SNRI coverage.
IP0477 (effective May 15, 2026) requires prior authorization for stimulants. SSRIs, SNRIs, bupropion, mirtazapine, and similar first-line antidepressants are typically unscheduled and are not governed by IP0477. Esketamine (Spravato) is Schedule III and often needs separate medical-benefit PA and REMS-certified administration. Confirm the current ESI list for your group number.
HHS/DEA telemedicine flexibilities for controlled substances currently run through December 31, 2026. Most oral antidepressants do not need those flexibilities. Benzodiazepines used as adjuncts are Schedule IV and may still need EPCS and extra PA.
When a Cigna fully insured plan covers an in-person depression visit, Texas Insurance Code Chapter 1455 generally requires comparable coverage for a qualifying telehealth visit, subject to network and documentation rules. Audio-only vs video, out-of-state licensure, and controlled-substance prescribing still follow federal and Texas medical board rules.
