UnitedHealthcare (UHC) members in California often ask whether depression evaluation, therapy, and antidepressants may be covered. Many commercial and Marketplace plans include mental health as an essential health benefit, and California’s mental health parity rules add extra duties for many state-regulated products. Coverage still varies by plan type, network, and the OptumRx formulary. Verify benefits before you book.

Often, yes, on many employer, individual, and Marketplace plans, when care is medically necessary and you use in-network benefits. Self-funded ERISA plans follow federal rules more than SB 855. Medicare Advantage and Medi-Cal use separate handbooks. Always check your Evidence of Coverage and call the number on your card.

SB 855 expanded California’s Mental Health Parity Act so many state-regulated plans must cover medically necessary treatment of mental health and substance use disorders, including depression, using generally accepted clinical standards. Medical necessity language lives in HSC § 1374.72 and Insurance Code § 10144.5. MHPAEA still bars more restrictive financial requirements or treatment limits for mental health than for medical/surgical care on many group plans.
Insurance Code § 10123.85 recognizes telehealth as a legitimate way to deliver covered services when the plan otherwise covers that service in person. That does not force every UHC product to pay every virtual visit at the same rate.
Most UHC commercial pharmacy benefits use OptumRx. Members can check coverage and costs in the OptumRx coverage tools. Generic SSRIs (for example sertraline, fluoxetine, escitalopram) and SNRIs (for example venlafaxine, duloxetine) often sit on preferred tiers. Newer agents, long-acting injectables, esketamine (Spravato), or combination products more often need prior authorization. Cigna’s IP0477 stimulant policy does not govern antidepressants.
CalPERS is a special case: as of January 1, 2026, CalPERS announced CVS Caremark replacing OptumRx as PBM for many members. If you are a CalPERS enrollee, use that card, not a generic UHC OptumRx assumption. (CalPERS plan changes)
A licensed clinician reviews history, current symptoms, safety, medications, and whether therapy, medication, or both may fit. If a prescription is appropriate, the clinician sends it to a pharmacy your plan may cover. Klarity visits are evaluations, not a promise that UHC will pay the claim.

Coverage varies by plan. Klarity cannot confirm your benefits. Use language like “Check if your plan may cover this,” not “Your insurance covers this.”
Many plans may cover FDA-approved antidepressants when they are on the OptumRx list and medically necessary. Tier, copay, and PA rules differ.
No. SB 855 and MHPAEA set parity and medical-necessity standards for many fully insured California products. They do not erase deductibles, networks, or documentation rules.
Often yes, when a licensed California clinician decides it is appropriate. DEA flexibilities through December 31, 2026 matter mainly if a controlled substance is involved. Typical SSRIs/SNRIs are not Schedule II–V.
Use the internal appeal in your EOC. Fully insured CDI products can involve CDI. Knox-Keene HMOs can request DMHC Independent Medical Review. Self-funded plans appeal through the employer and federal process.
No. Those programs have their own formularies and appeal paths. Do not assume commercial UHC rules apply.
