Anthem Blue Cross in California may help pay for medically necessary weight management care on some commercial plans. Coverage is not automatic. GLP-1 drugs, phentermine, Contrave, orlistat, and nutrition counseling sit on different Carelon Rx lists, tiers, and prior-authorization rules. California mental health parity under SB 855 and Health and Safety Code § 1374.72 typically applies to mental health and substance use, not obesity medications.   • Phentermine and Qsymia are Schedule IV. GLP-1s, Contrave, and orlistat are not scheduled.
  • Medi-Cal and Medicare Advantage follow different rules than fully insured commercial Anthem PPO/HMO products.

Does Anthem typically cover weight loss treatment in California?

Many Anthem Blue Cross of California commercial plans may cover evaluation, follow-up, and certain anti-obesity medications when criteria are met. Plans often require a BMI threshold, documented lifestyle attempts, and prior authorization. Employer carve-outs, ACA metal-level products, and self-funded ERISA groups can exclude weight-loss drugs even when they cover diabetes GLP-1s. Always confirm with Anthem member services and your Summary of Benefits.

Check if your plan may cover online weight loss care

SB 855 does not force GLP-1 coverage

Federal MHPAEA and California SB 855 (HSC § 1374.72 / Insurance Code § 10144.5) generally require comparable treatment of mental health and substance use benefits. Obesity pharmacotherapy is typically a medical/pharmacy benefit, not a parity mandate. Telehealth in California

Insurance Code § 10123.85 and related Health and Safety Code telehealth rules generally require coverage of otherwise covered services when they are appropriately delivered by telehealth, subject to the contract. That does not create a new drug benefit. If the visit is covered in person, a comparable telehealth visit may be covered. HHS/DEA telemedicine flexibilities for some controlled substances currently extend through December 31, 2026; phentermine and Qsymia remain Schedule IV and follow DEA and plan rules.

Carelon Rx, GLP-1s, and other medications

Anthem California pharmacy is commonly Carelon Rx. Log in and search the California drug list (use the official Anthem CA formulary page: anthem.com/ca pharmacy formulary). Prior authorization pages for California prescribers live at Anthem CA Rx PA.

  • Wegovy, Zepbound, and similar GLP-1s for weight loss often need PA, step therapy, and quantity limits when they appear on a commercial list at all.
  • Caremark’s July 2025 Zepbound removal and October 1, 2026 reinstatement apply to many Aetna/CVS lists, not automatically to Carelon.
  • Cigna/ESI often kept Zepbound on Tier 3-4. That is a Cigna differentiator, not an Anthem fact.
  • Contrave and orlistat may appear as non-scheduled options on some lists.
  • Phentermine/Qsymia: Schedule IV; EPCS and PDMP rules may apply.

Medicare GLP-1 Bridge vs Anthem commercial

Starting July 1, 2026, CMS operates the Medicare GLP-1 Bridge so eligible beneficiaries may access Foundayo, Wegovy, and Zepbound at $50 per month through December 31, 2027 in the demonstration design described by CMS. Read CMS, the Medicare fact sheet, and Medicare.gov weight-loss drugs. This is not your Anthem commercial Carelon formulary. Dual-eligible and Medi-Cal members should not assume commercial Anthem rules apply.

How to verify Anthem California benefits

  1. Call the number on your Anthem card. Ask whether anti-obesity medications and telehealth obesity visits are in-network benefits.
  2. Pull the current Carelon/Anthem CA formulary for your group number.
  3. Ask about BMI, comorbidity, duration of lifestyle documentation, and PA forms.
  4. If you have a Knox-Keene HMO/HP, DMHC may be the regulator. PPO/insured products often go to CDI at 1-800-927-4357.
  5. Self-funded employer plans follow ERISA and the employer’s SPD, not CDI in many cases.
