What is Jornay PM

What is Jornay PM?

Jornay PM (methylphenidate) is a unique central nervous (CNS) stimulant medication taken at night to manage attention-deficit/hyperactivity disorder (ADHD) symptoms through the following day. It was approved by the U.S. Food and Drug Administration (FDA) in 2018 for the treatment of ADHD in individuals aged 6 years and older, including adults. 

Like other stimulants for ADHD, Jornay PM’s active ingredient is methylphenidate. But generic methylphenidate and Jornay PM aren’t the same medication. Instead, Jornay PM is uniquely formulated to be both a delayed-release and extended-release medication. Its formulation starts working in the morning to help reduce common ADHD symptoms, like impulsivity, inattention, and hyperactivity, for up to 12 hours without the need for morning dosing or multiple doses throughout the day. 

It’s an oral capsule.

How does Jornay PM work for ADHD?

Methylphenidate in Jornay PM helps increase the neurotransmitters dopamine and norepinephrine in the brain. Dopamine and norepinephrine are chemical messengers that carry signals between brain cells. Dopamine affects concentration, mood, memory, motivation, and more. Norepinephrine works on regulating attention and cognitive function.

Higher levels of dopamine and norepinephrine help improve communication between brain cells, which helps with enhanced focus, attention, and impulse control for people with ADHD.

Jornay PM uses a unique delivery method. Each capsule has 2 layers. The outside layer delays the medication’s release for 10 to 12 hours after a dose is taken. The inside layer helps part of the drug gradually be released over the 12 hours after that to extend its effects into the evening.

Who can take Jornay PM for ADHD?

Jornay PM is FDA-approved for the treatment of ADHD in children ages 6 and older, including adults.

How to get a Jornay PM prescription online

A licensed, board-certified healthcare provider experienced in managing ADHD and qualified to prescribe medications is the first step in getting a Jornay PM prescription. This can include a primary care provider (PCP and MD), physician’s assistant/associate (PA), nurse practitioner (NP), psychiatrist (MD), or psychiatric nurse practitioner (PNP).

How to use Jornay PM and Jornay PM dosages

In individuals 6 years of age and older, Jornay PM is a capsule taken in the evening initially at 8:00 p.m. Depending on how you respond to it, your provider may have you adjust the time you take your dose to anywhere between 6:30 p.m. and 9:30 p.m. Once you know the best time to take Jornay PM for you, you’ll take it consistently at the same time each evening. 

You want to swallow the capsule whole, not crush or chew it, to maintain its delayed-release effect. You can take Jornay PM with or without food, but want to take it consistently with or without food each day. 

If you have difficulty swallowing capsules, you can use the sprinkle method to take Jornay PM. Open it and sprinkle the contents onto applesauce, pudding, or something similar. Take the entire contents of the capsule on the applesauce at once and immediately. Swallow the contents, which resemble beads, whole. Don’t chew them.  

Jornay PM is available in 20-, 40-, 60-, 80-, and 100-milligram capsules. Your provider will typically have you start with 20-milligram increments. If needed, you can increase your dose 20 milligrams a week up to a maximum daily dose of 100 milligrams.  

If you miss taking your dose of Jornay PM at your regularly scheduled time, you should take it as soon as you remember that same evening. If you don’t remember until the following day, skip your missed dose and take your next dose at the normal evening time. 

If you need to discontinue Jornay PM, your provider will have you gradually decrease your daily dosage by 20 milligrams a week to help prevent withdrawal symptoms.

Jornay PM side effects and warnings

Jornay PM can cause side effects but they’re often mild and may subside with time. Common side effects can include decreased appetite, trouble sleeping, nausea, vomiting, indigestion, stomach pain, headache, weight loss, anxiety, dizziness, irritability, mood swings (affect lability), increased heart rate, and increased blood pressure.

Less common, and more serious side effects, can include a risk of sudden death in patients with severe cardiac disease, painful and prolonged erections (priapism), circulation problems in your fingers and toes (peripheral vasculopathy, including Raynaud’s phenomenon), slowed growth (height and weight) in children, eye problems (increased pressure in the eye and glaucoma), and new or worsening tics or worsening Tourette syndrome and other mental health problems. 

Jornay PM is also a controlled substance and has an FDA-boxed warning for the risk of dependence, abuse, and misuse. If you have a history of substance use disorder, discuss the risks versus benefits of taking Jornay PM with your provider.

What does Jornay PM cost?

According to drugs.com and with its discount, the cost of Jornay PM in any strength, 20 milligrams to 100 milligrams, is about $1,609 for 100 capsules. That’s a cost of $16.10 per pill. There is no generic version of Jornay PM available. Generic methylphenidate is not the same as Jornay PM.

Jornay PM’s manufacturer, Collegium Pharmaceutical., does offer a savings card that lets people with health insurance pay as little as $0 for their first prescription. People whose insurance covers Jornay PM can pay no more than $25 for refills while people whose insurance doesn’t cover it can pay no more than $75 per refill.

How does Jornay PM compare to other ADHD medications?

Many ADHD medications are central nervous system (CNS) stimulants, which are proven effective and serve as a first-line ADHD treatment for many individuals. Jornay PM is a stimulant that shares the active ingredient methylphenidate with other medications like Ritalin (methylphenidate) and Concerta (methylphenidate). 

Jornay PM’s unique delayed and extended-release tablet formulation set it apart from other methylphenidate-based medications. Ritalin is available in both immediate-release and extended-release tablets (Ritalin LA). 

Adderall (amphetamine/dextroamphetamine), the most widely used CNS stimulant for ADHD, contains a mix of amphetamine salts and is available in brand-name and generic immediate and extended-release forms (Adderall XR). Adderall XR can affect symptoms for up to 12 hours, but it doesn’t come in a formulation taken at night. 

There is a medication named Mydayis which uses the same active ingredient as Adderall, amphetamine/dextroamphetamine. It’s taken in the morning and can last for up to 16 hours.  

Non-stimulant options, such as Strattera (atomoxetine) and Qelbree (viloxazine) are selective norepinephrine reuptake inhibitors (NRIs). They offer an alternative to stimulants and can have fewer stimulant-related side effects. And, because they aren’t stimulants, which are controlled substances, they aren’t known to lead to abuse, misuse or addition, which may make them ideal for someone with a substance use problem.

Ritalin, Concerta, atomoxetine, Qelbree, and Mydasis—along with their generic counterparts when available — are FDA-approved medications for treating ADHD in adults. They may cause side effects, such as insomnia, reduced appetite, nausea, and irritability. Ritalin, Concerta, and Mydasis are controlled substances and can lead to an increased heart rate, misuse, abuse, and dependency.

Ultimately, which ADHD medication works best for you, depends on your unique physiology and needs. It can take some trial and error for you and your provider to determine which medication will work best for your ADHD symptoms. Learn more about ADHD medications.
