Symptoms of HPV

HPV infections often go unnoticed because they may not cause symptoms. However, persistent infections can lead to visible or serious health issues. Different HPV strains produce different effects on the body.

Common symptoms of HPV:

  • Genital warts
    • Flesh-colored or gray growths in the genital, anal, or inner thigh areas
    • May occur singly or in clusters; typically painless, but may itch or bleed
    • Caused by low-risk HPV types (especially types 6 and 11)
  • Respiratory papillomatosis
    • Rare condition involving wart-like growths in the throat or larynx
    • Can cause hoarseness, chronic cough, or airway blockage
    • May occur in children exposed to HPV during childbirth
  • Cervical dysplasia (abnormal cervical cells)
    • Usually asymptomatic
    • Detected through Pap tests or HPV deoxyribonucleic acid (DNA) testing
    • High-grade lesions (caused by types 16 and 18) can progress to cervical cancer if untreated
  • HPV-related cancers
    • Can affect the cervix, anus, vulva, vagina, penis, and oropharynx (throat)
    • May cause pelvic pain, unusual bleeding, persistent throat soreness, or painful urination in late stages
  • Anal and penile lesions
    • Flat or raised areas with discoloration, skin thickening, or irritation
  • Subclinical HPV
    • No visible symptoms but detectable through screening
    • Common among individuals with a cervix who test positive for high-risk strains

Because HPV can remain dormant for years, regular screening is critical — especially for individuals who are sexually active, immunocompromised, or have a history of abnormal test results.

How is HPV diagnosed and tested?

HPV diagnosis depends on the individual’s sex, anatomy, age, and medical history. Routine testing is currently only recommended for individuals with a cervix, but men may undergo examination when symptoms are present.

Diagnostic methods include:

  • Pap smear
    • Detects abnormal cervical cells (dysplasia or precancerous changes)
    • Recommended every 3 years for individuals aged 21 to 29. For those aged 30 to 65, options include: Pap test every 3 years; High-risk HPV (hrHPV) testing every 5 years; Co-testing (Pap test combined with hrHPV testing) every 5 years
  • HPV DNA test
    • Identifies high-risk HPV types (e.g., 16, 18) in cervical cells
    • Used alone or with a Pap test (co-testing) for individuals aged 30 to 65
  • Reflex HPV testing
    • Conducted after an abnormal Pap smear to detect high-risk strains
  • Colposcopy and biopsy
    • A magnified examination of the cervix or genital tissue, often with a tissue sample taken for histology
  • Visual inspection
    • Used to diagnose external genital or anal warts based on appearance
  • At-home HPV test kits
    • Self-collected cervical or vaginal samples mailed to a lab for DNA analysis
    • Available for individuals with a cervix. While they can increase screening rates among those overdue for screening, they are not a replacement for regular in-clinic screenings.
  • Anal Pap test (off-label)
    • Sometimes used in high-risk groups, such as men who have sex with men or people living with human immunodeficiency virus (HIV)
  • HPV testing for men
    • No U.S. Food and Drug Administration (FDA)-approved screening test exists.
    • Genital warts are usually diagnosed visually by a healthcare provider.

HPV treatment options

HPV can’t be cured, but its complications—like genital warts and abnormal cell growth—can be effectively managed. Treatment plans vary by symptom severity, affected area, and patient preferences.

Genital warts treatment

  • Topical prescription medications
    • Imiquimod (Aldara, Zyclara) – Immune response modifier applied several times per week
    • Podofilox (Condylox) – Causes wart tissue breakdown; applied twice daily in treatment cycles
    • Sinecatechins (Veregen) – Green tea extract ointment with antiviral and antioxidant properties
  • In-office provider-administered procedures
    • Cryotherapy – Freezes off warts using liquid nitrogen
    • Trichloroacetic acid (TCA) – Chemical cauterization used on external and internal warts
    • Electrosurgery or excision – Reserved for large, recurrent, or resistant warts

Cervical dysplasia management

  • Observation – Low-grade lesions may be monitored, especially in younger individuals
  • Colposcopy and biopsy – For more detailed evaluation and diagnosis
  • Loop electrosurgical excision procedure (LEEP) – Loop electrosurgical excision of abnormal tissue
  • Cold knife cone biopsy – Surgical removal of deeper cervical lesions under anesthesia

These methods aim to prevent a progression to cervical cancer while preserving healthy tissue.

HPV vaccination

  • Gardasil 9
    • FDA-approved to prevent infection from 9nine HPV strains, including types 6, 11, 16, and 18
    • FDA-approved for individuals aged 9 through 45 years to prevent diseases caused by HPV types 6, 11, 16, 18, 31, 33, 45, 52, and 58, including cervical, vulvar, vaginal, anal, oropharyngeal, and other head and neck cancers, as well as genital warts
    • Prevents most HPV-related genital warts and cancers, but does not treat existing infections
