NOTE: No Anthem or Blue Cross/Blue Shield MEDICAID/MEDICARE plans. Regular Anthem and Blue Cross and Blue Shield plans are GOOD. Other Medicaid/Medicare plans may be appropriate.

Controlled Substances: Please note, BEST PRACTICE requires that any prescription of a controlled substance or any therapy is based on a comprehensive evaluation; taking into account your past medical history, any necessary objective testing, and your presenting symptoms. Booking an appointment with me does not guarantee any particular medication or treatment. Thank you for understanding.
