Lybalvi (olanzapine and samidorphan) is a once-daily tablet that pairs an atypical antipsychotic with an opioid blocker. Clinicians use it for schizophrenia in adults and for bipolar I mania or mixed episodes, including maintenance and use with lithium or valproate. Samidorphan is there to blunt some of the weight gain that often comes with olanzapine. It is not a diet pill, and it is not interchangeable with Zyprexa. Because samidorphan blocks opioid receptors, you cannot start Lybalvi if you currently use opioids or are in acute opioid withdrawal.

Olanzapine by itself is the medicine in Zyprexa. MedlinePlus describes olanzapine as an atypical antipsychotic used for schizophrenia and bipolar I. It changes the activity of certain natural substances in the brain. Lybalvi is a different product because of samidorphan. NAMI describes olanzapine as a second-generation antipsychotic that rebalances dopamine and serotonin to improve thinking, mood, and behavior. Samidorphan is an opioid antagonist. It may help reduce weight gain that olanzapine can cause. It does not cancel every metabolic risk. You still need weight, glucose, and lipid checks. (NAMI; DailyMed metabolic warnings)

Always follow the dose on your prescription. DailyMed recommended patterns:
Take once daily with or without food. Do not divide tablets or combine strengths. NAMI repeats the same range and says only your clinician can set the dose. If you miss a dose, take it when you remember unless it is close to the next dose. Do not double up. (DailyMed; NAMI)
DailyMed lists the most common schizophrenia reactions (incidence ≥5% and at least twice placebo) as weight increased, somnolence, dry mouth, and headache. For olanzapine in bipolar mania, common reactions include somnolence, dry mouth, dizziness, asthenia, constipation, dyspepsia, increased appetite, and tremor. Adjunct mania lists also include back pain, speech disorder, increased salivation, amnesia, and paresthesia. (DailyMed)
NAMI also lists low blood pressure, extrapyramidal symptoms, restlessness, insomnia, higher prolactin, higher glucose and triglycerides, liver-test changes, and weakness among common effects. Serious risks include opioid withdrawal if opioids are still in your system, tardive dyskinesia, neuroleptic malignant syndrome (fever, rigidity, confusion, sweating; NAMI cites <1%), falls, and low neutrophils. DailyMed also warns about DRESS (rash with fever, eosinophilia, and organ involvement), seizures, swallowing problems, heat dysregulation, anticholinergic effects, and high prolactin. In the 4-week trial, shifts from normal to high prolactin occurred in 41.4% of females and 32.9% of males on Lybalvi. Somnolence occurred in 9% on Lybalvi vs 2.2% on placebo. (DailyMed)
Monitor fasting glucose and lipids at the start and periodically. Weigh yourself often. Call your clinician for extreme thirst, frequent urination, unexplained weight change, fever with stiff muscles, new rash, or movements you cannot control.
Boxed warning. Older adults with dementia-related psychosis who take antipsychotics have a higher risk of death. Causes in studies were often cardiac events or infection such as pneumonia. Lybalvi is not approved for that use. Stroke and mini-stroke risk is also higher in this group. (DailyMed; MedlinePlus)
Opioids. Do not take Lybalvi if you use opioids (including some cough syrups with codeine) or if you are in acute opioid withdrawal. Samidorphan can trigger severe withdrawal. NAMI states there should be at least a 7-14 day opioid-free period before the first dose. If you stop Lybalvi, your opioid tolerance may drop. Restarting an old opioid dose can be fatal. Do not try to “break through” the blockade with extra opioids. Emergency opioid anesthesia needs trained staff, airway support, and usually a stop of Lybalvi. Other limits. DailyMed does not recommend Lybalvi in end-stage renal disease. Third-trimester antipsychotic exposure can cause EPS or withdrawal in newborns. Olanzapine passes into breast milk; whether samidorphan does is not fully known. Discuss pregnancy and feeding plans with your clinician. (DailyMed; NAMI)

A licensed telehealth clinician can often take a full history, review current medicines and opioid use, order metabolic labs, and continue a stable Lybalvi plan. Video follow-up works well for side-effect checks, smoking-status changes, and dose questions once the chart is complete.
In-person or emergency care is the safer path for new or worsening mania with safety risk, suspected NMS or DRESS, severe opioid withdrawal, fainting, seizure, aspiration, or overdose. Tell every surgeon and dentist you take Lybalvi before procedures that might need opioids.
