About phendimetrazine

About phendimetrazine

What is phendimetrazine?
Phendimetrazine is a stimulant medication used as an appetite suppressant. It’s approved by the U.S. Food and Drug Administration (FDA) short-term use (a few weeks) to help with weight management in people with a BMI of 30 or higher, or 27 or higher if they have other health risks like high blood pressure, diabetes, or high cholesterol—and only if diet and exercise alone haven’t worked.

Brand names included Bontril PDM, Bontril Slow-Release, and Melfiat, but they have been discontinued in the US.

Phendimetrazine works by stimulating your central nervous system to reduce appetite and increase energy, which can make it easier to stick to a diet.  In one study, 61.2% of participants taking phendimetrazine lost an average of 1.9 pounds per week. Phendimetrazine is usually prescribed as part of a medically supervised weight loss plan that includes diet and exercise.

It comes in both immediate-release and extended-release (ER) tablets or capsules.

How to get a phendimetrazine online prescription?

Getting phendimetrazine online is simple with telehealth platforms like Klarity Health. Here’s how it works:

  • Step 1: Start by filling out a quick medical questionnaire online
  • Step 2: Get matched with a board-certified provider  such as a medical doctor (MD), nurse practitioner (NP), or physician assistant (PA)
  • Step 3: Attend a virtual appointment to discuss your weight loss goals and health history
  • Step 4: If phendimetrazine is medically appropriate, your provider will send the prescription to a local or online pharmacy of your choice

No insurance is required, and appointments are fast, confidential, and affordable.

Phendimetrazine dosages and how to use it

Phendimetrazine dosages and how to use it

Phendimetrazine comes in two main forms:
• Immediate-release tablets (35 milligrams)
• Extended-release capsules (105 milligrams )

Typical dosing guidelines:
• Immediate-release: 1 tablet (35 milligrams) taken 2 or 3 times a day, 1 hour before meals.
• Extended-release: 1 capsule (105 milligrams) once daily, 30 to 60 minutes before breakfast

Your dose of immediate-release phendimetrazine should be based on how well it manages your symptoms. Your provider will try to get you to the lowest effective dose, which can be as little as half a tablet (17.5 milligrams) per dose (2  to 3 doses each day).

The maximum daily dose is 2 tablets 3 times a day.
The maximum daily dose of extended-release phendimetrazine is 1,105 milligram capsule per day.

Important tips for taking phendimetrazine:
• Do not crush, chew, or split extended-release capsules
• Take earlier in the day to avoid insomnia
• Do not exceed 210 mg per day (immediate-release)
• If you miss a dose, skip it; do not double up
• If you need to stop taking phendimetrazine, your doctor may gradually decrease your dose

Phendimetrazine can interact with several medications and substances. It should not be taken with monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping them, due to the risk of a dangerous spike in blood pressure. Drinking alcohol while taking phendimetrazine may lead to adverse reactions. It may also alter how your body responds to insulin or oral diabetes medications, and can reduce the effectiveness of drugs used to lower blood pressure, like adrenergic neuron blockers. Phendimetrazine is labeled a Schedule III controlled substance because of its risk of misuse and dependence.

How does phendimetrazine compare to other medications?

Compared to other stimulant weight loss medications, phendimetrazine is similar in function to phentermine, benzphetamine (Didrex), and diethylpropion (Tenuate). These medications all act as appetite suppressants and are FDA-approved for short-term use in individuals with obesity or overweight with risk factors. Phendimetrazine is unique because it’s available in both immediate-release and extended-release forms, allowing for flexible dosing schedules. Like others in this class, it’s typically prescribed for just a few weeks due to the risk of dependence and potential side effects like insomnia, dry mouth, and elevated blood pressure.

Compared to newer weight loss medications like semaglutide (Ozempic, Wegovy), phendimetrazine usually works faster but is generally less effective for long-term weight management. Semaglutides usually have gradual weight loss, but are meant to be longer-term solutions. One clinical trial reported that semaglutide helps patients lose more weight over time,18.5% of their total starting body weight in 68 weeks. But this is only 0.39 pounds per week compared to the average 1.9 pounds per week typically seen with phendimetrazine. While phendimetrazine suppresses appetite by stimulating the central nervous system (CNS), semaglutide mimics the glucagon-like peptide-1 (GLP-1) hormone, which regulates appetite and blood sugar. Semaglutides can have side effects such as stomach or bowel problems.

Phendimetrazine also differs from glucagon-like peptide-1 (GLP-1) receptor agonists like liraglutide (Saxenda) and tirzepatide (Zepbound), which are also designed for longer-term use alongside diet and exercise. These medications not only reduce appetite but also improve blood sugar control, especially in patients with type 2 diabetes. They tend to have fewer stimulant-related side effects, but they do have their own side effects, like nausea and diarrhea. While GLP-1s are generally more effective for sustained weight loss, phendimetrazine may be considered for short-term jumpstarts or in patients who can’t tolerate GLP-1 therapy.
